Deerfield.com Tax Exempt


All Michigan organizations that qualify for tax-exempt status will need to fill out the following tax-exempt form. Please print the form, sign it, and fax it to our Sales Dept. at (989) 731-2642. Once we receive the proof of your tax-exempt status, your order will be processed promptly.

Note: This form must be on file in order to receive your tax-exempt status on any Deerfield.com products. Additionally, if you are a non-profit organization and wish to receive our non-profit discount, this form must be filled out and faxed back to us before your order can be properly processed.
